How to get your Personal Wellness Profile:

 

Read an overview of the wellness approach:  Wellness Step-by-Step (PDF).      Look at an example (PDF) of what your Profile will look like.

If necessary, review the various Profiles, select one and pay for it.

If you want a physical examination, print and fill out the Pre-visit questionnaire(PDF) and the Medical Examination Report
and take these to your doctor.
 

1) Examine your Lifestyle:

       Complete the on-line Lifestyle Evaluation Questionnaire

2) Order your Blood Tests:

       Have a look at the Laboratory tests for your Profile – you can read explanatory notes on each one, if you like.

Click here to print the Laboratory Order Form  and take it to the Laboratory.

Alternatively, if you already have the following lab test results, enter them on-line here:  Laboratory Test Results
(Blood Pressure, Total cholesterol, HDL, LDL, Triglycerides, Fasting Blood Glucose, PSA, Faecal Occult Blood - at least the first two are required)

 

3) Do your Home Fitness Assessment:  (Optional, but strongly recommended)

       Print and follow the Instructions for your Fitness Assessment;
       then submit your results on-line

      

4) Your Personal Wellness Profile will be delivered by e-mail within two weeks.

 

This is YOUR report; a  close look at your personal health in several dimensions. Unlike a medical report, it is not about what is wrong with you, but about what you have to DO in order to enjoy higher standards of health and well-being. It is a guide and a reference that you will want to return to for many years to come.

Specific targets may have been set for the next few months. Like all the figures in your report, these should be interpreted as a guide. Adjust them if necessary to give yourself a realistic chance of achieving your objectives.

Your Personal Wellness Profile is written in language you can easily understand, and it will be very clear what you have to do in order to achieve your wellness goals. However, you may still wish to make an appointment to discuss your profile with the doctor.

Progress Report
After 3 to 6 months, you should repeat your Personal Wellness Profile. A Progress Report can then
be prepared (
sample
(PDF)) indicating those areas in which you have improved, and those in which further effort is needed.

 

The Wellness Community
If you have not already done so, you are invited to join the Wellness Community.

The Wellness Community will open up many new opportunities for you to be proactive about your health; to share your ideas and experiences, to find information and get support in dealing with your particular health challenges. You can even set up your own group of friends to work together on your mutual goals and interests.
